SUMMARY
Under general supervision, is responsible for the development and
implementation of the Individualized Service Plans for individuals
and their advocates in accordance with the regulations and
requirements of the applicable governmental and other regulatory
agencies within a residential setting. Provides case management
services to assigned individuals and determines individual
aptitudes, interests and preferences. Identifies appropriate
service providers and links individuals with providers. Coordinates
services and monitors individual's satisfaction with the
services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Case Manager will include,
but are not limited to the following:
- Provides social services to each person we support directed
toward maximizing their social functioning, enhancing coping
capacity of his/her family, asserting and safeguarding their human
and civil rights and those of his/her family and fostering their
human dignity and personal worth.
- Develops and monitors individual treatment plan in accordance
with regulatory and agency policy.
- Participates in interdisciplinary planning. Interprets,
documents and follows up on all recommendations of
interdisciplinary team meetings. Chairs all meetings of the
interdisciplinary team.
- Includes channels of communications and consultation with
residential staff and professional staff to interpret and explain
the psychological needs of the people we support as they progress
through habilitation.
- Develops goals and activities generated by interdisciplinary
team. Produces an active treatment plan schedule for each
individual served.
- Documents comprehensive functional assessment and review of
plan of care as needed.
- Ensures proper data collection through observation and
in-service training.
- Assists in all activities of daily living and participates in
activities outside of residence, i.e. shopping, trips swimming pool
as needed.
- Supervises Residence Program Specialists in implementation of
active treatment plans.
- Makes regular and meaningful family, resident and agency-wide
contacts.
- Trains and instructs Residence Program Specialists on
individual's specific goal plans, relevant data collection, daily
routine and general procedures and practices of the residence.
- Maintains contacts with government social services agencies,
not-for-profit social service agencies to access resources and/or
advocate for services.
- Performs individual evaluations to determine whether and when
alternate placement is needed.
- Handles family outreach and in-service training.
- Ensures that all benefits for which the people we support may
be eligible for are applied for in an expeditious manner and to
effect transfer of any prior accumulated funds to the agency.
- Provides systematic follow-up to assure referral to appropriate
community agencies upon discharge.
- Works regularly with, develops, or is responsible for
maintaining security of confidential information such as records of
person receiving services which requires regular use of
discretion.
- Ensures all adaptive equipment is secured and maintained in
good working order.
- Advocates for the people we support to ensure that all of their
rights are being fulfilled.
- Writes monthly case notes on all people we support's
interpersonal, social, workshop progress, and family contacts.
- Serves as the liaison to Day Programs, Service Coordination,
and Clinics as needed.
- Keeps abreast of changes in the field.
- Performs other related duties as requested. QUALIFICATIONS
To perform this job successfully, an individual must be able to
perform each essential duty satisfactorily. The requirements listed

- Ability to read, write, speak and understand English.
- Good oral and written communication skills.
- Excellent organizational skills.
- Ability to read, write, speak and understand English.
- Good interpersonal skills necessary to interact effectively
with coworkers, employees, the people we support, and their
families.
- Must be able to maintain the strictest confidentiality.
- Proficiency in the use of a personal computer and appropriate
software and E-Mail system.
- Knowledge of equipment typically used by persons receiving
services including wheelchairs and adaptive devices.
- Ability to analyze problems and determine corrective
measures.
- Ability to work flexible hours. EDUCATION and/or EXPERIENCE
- Bachelor's Degree in the Human Services field.
- At least one year of prior experience working with people with
developmental disabilities.
COMPENSATION: $26.78 - $27.81/hour (after probation)
